Elite Home Health is seeking a Licensed Physical Therapist Assistant (PTA) to provide skilled physical therapy in patients' homes under the supervision of a licensed Physical Therapist. This role emphasizes collaborative care, safety, and adherence to the established plan of care in a Florida home health setting.

Qualifications include a PTA program graduate with active Florida licensure, CPR certification, and a minimum of one year of PT experience (home health preferred).

Qualifications

• Graduate of an accredited Physical Therapist Assistant (PTA) program required.
• Current state PTA licensure or certification in good standing required.
• Minimum of one year of experience in physical therapy or related clinical setting; home health experience preferred.
• Current CPR certification required.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to pass background screening and all compliance requirements.

Responsabilités

• Provide skilled physical therapy services under the supervision of a licensed PT.
• Deliver therapy interventions per physician orders and established plan of care.
• Document treatment and progress in the EMR system timely and accurately.
• Coordinate with nurses, physicians, and care team to support continuity of care.
• Participate in care conferences and discharge planning.
