Elite Home Health in Crystal River, FL is hiring a Licensed Physical Therapist Assistant (PTA) to provide skilled therapy in patients' homes under the supervision of a licensed PT. This PRN home health role requires an active Florida PTA license and CPR certification, serving patients in Crystal River and surrounding areas.

The PTA will support mobility, independence, and safe functional movement, documenting care and coordinating with the interdisciplinary team.

Qualifications

• Graduate of an accredited Physical Therapist Assistant program.
• Current Florida PTA license/certification in good standing.
• Current CPR certification.
• Valid driver's license and reliable transportation.
• Previous clinical experience preferred; home health experience is a plus.

Responsibilities

• Provide skilled PT treatments under supervision of the PT.
• Assist patients with therapeutic exercises, gait training, transfer training, and mobility.
• Support strengthening, fall prevention, and safe functional movement.
• Monitor patient response and report progress to the PT.
• Document accurately in the EMR and communicate with the care team.
• Participate in care coordination and required trainings.
• Follow Medicare/state regulations and agency policies.

Position Summary

The PTA provides skilled physical therapy treatments in accordance with physician orders, the established therapy plan of care, PT direction, agency policies, and applicable regulations.

Responsibilities

• Provide physical therapy treatments under the direction of the supervising PT
• Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility training
• Support strengthening, fall prevention, and safe functional movement
• Monitor patient response to treatment and report progress or concerns to the PT
• Reinforce patient and caregiver education as directed by the PT
• Complete accurate and timely documentation in the EMR
• Communicate with the PT and interdisciplinary care team regarding patient needs
• Participate in care coordination, meetings, in-services, and required training
• Follow Medicare requirements, state regulations, and agency policies

Qualifications

• Graduate of an accredited Physical Therapist Assistant program
• Current Florida PTA license/certification in good standing
• Previous clinical experience preferred; home health experience a plus
• Current CPR certification
• Valid driver's license, reliable transportation, and auto insurance
• Ability to meet all required background and compliance requirements
