Job summary

Elite Home Health Group INC is seeking a Licensed Physical Therapist Assistant (PTA) to provide skilled therapy services to patients in their homes across Spring Hill, FL. The PTA will operate under the direction of a licensed Physical Therapist and collaborate with an interdisciplinary team to deliver patient-centered care.

The role requires a Florida PTA license, CPR certification, and reliable transportation.

Qualifications

Graduate of an accredited Physical Therapist Assistant program.
Current Florida PTA license in good standing.
Current CPR certification.
Valid driver's license, reliable transportation, and auto insurance.
Ability to meet all required background and compliance requirements.

Responsibilities

Provide physical therapy treatments under the direction of the supervising PT.
Assist patients with therapeutic exercises, gait training, transfer training, balance activities, and mobility training.
Support strengthening, fall prevention, and safe functional movement.
Monitor patient response to treatment and report progress or concerns to the PT.
Reinforce patient and caregiver education as directed by the PT.
Complete accurate and timely documentation in the EMR.
Communicate with the PT and interdisciplinary care team regarding patient needs.
Participate in care coordination, meetings, in-services, and required training.
Follow Medicare requirements, state regulations, and agency policies.
