Elite Home Health in Crystal River, FL is seeking a PRN Licensed Physical Therapist (PT) to provide skilled therapy services to patients in their homes throughout Crystal River and surrounding areas. The role requires an active Florida PT license and CPR certification, with a strong emphasis on home-based care.

The position offers compensation ranging from $75.00 to $130.00 per hour, with duties including evaluations, exercise programs, gait and balance training, patient and caregiver education,

Competenze

• Graduate of an accredited Physical Therapy program.
• Current Florida PT license in good standing.
• Minimum 1 year of PT experience preferred.
• Current CPR certification.
• Valid driver's license, reliable transportation, and auto insurance.
• Ability to meet all required background and compliance requirements.

Mansioni

• Perform physical therapy evaluations and develop individualized treatment plans
• Provide skilled therapy, including therapeutic exercises, gait training, balance training, and mobility interventions
• Educate patients and caregivers on exercises, safety, fall prevention, and assistive devices
• Monitor patient progress and adjust treatment plans as needed
• Complete timely and accurate clinical documentation
• Communicate with physicians, nurses, and the interdisciplinary care team
• Supervise PTAs as required
