Job summary

SunCrest Home Health, part of the LHC Group and Optum family, seeks a Physical Therapist Assistant to deliver treatments and patient education per the plan of care. You will enable improved well-being through prescribed exercises and education in the home setting.

Primary responsibilities include following state PT laws, guiding patients in exercises, documenting responses to treatment, and coordinating with the care team. Weekend rotation and valid driver’s license are required.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• CPR certification is preferred or can be completed within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state-specific laws governing the provision of physical therapy in home care; perform only procedures qualified and trained to perform; suggest changes to plan of care to supervising PT.
• Instruct and aid patients in active and passive exercise, muscle re-education, gait, functional, ADL, transfer, safety, and prosthetic training.
• Observe, record, and report patient response to treatment; coordinate care with the health care team.
• Instruct patient, family, caregiver, and health care team in therapy within PTA scope.
• Function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.
