Job summary

SunCrest Home Health, a part of the LHC Group and Optum family, is seeking a Physical Therapist Assistant to implement treatment plans, provide exercise instruction, and educate patients to support their well-being in home care.

You will follow the supervising PT's plan of care, assist with gait and mobility, document responses, and coordinate with the care team. Qualified candidates must be licensed PTA, have a valid driver's license, and be willing to work weekends.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state-specific laws governing the provision of physical therapy in home care and adhere to the supervising PT plan of care.
• Instructs and aids patients in active and passive exercise, gait, ADL, transfer, and prosthetic training.
• Observe, record, and report patient responses and coordinate care with the health care team.
