Suncrest Home Health, part of the Optum family, is seeking a Per Diem Physical Therapist Assistant to provide therapy services in home health settings under the supervision of a PT. The role involves implementing the plan of care, educating patients and families, and coordinating with the healthcare team.

Applicants must hold a current PT license in the state, have a valid driver's license, be able to work weekends, and obtain CPR certification within 90 days.

Qualifikationen

• Licensed to practice physical therapy in state of practice.
• Valid driver’s license and access to a vehicle or public transport.
• Available to work weekend rotations.
• CPR certification or willingness to obtain within 90 days of hire.
• Ability to perform patient-care tasks including lifting and transfers.

Aufgaben

• Follow state-specific laws and execute the plan of care under supervising PT.
• Instruct patients in active/passive exercise, gait, ADLs, and mobility.
• Observe and report patient responses; coordinate with the healthcare team.
• Educate patient, family, and care team in therapy within the PTA scope.
• Function in diverse home situations regardless of background.
