SunCrest Home Health, a part of LHC Group and member of the Optum family, seeks a Physical Therapist Assistant to implement treatment plans and educate patients to improve well-being. The role requires working within plan of care under supervising PT, and coordinating with the care team in a home health setting.

Responsibilities include guiding exercises, monitoring patient progress, and training family or caregivers.

Qualifications

• Current, licensed to practice physical therapy in state of practice.
• Valid driver's license and vehicle insurance, or access to reliable transportation.
• Available to work weekend rotation.

Responsabilités

• Follow state-specific laws governing provision of physical therapy in home care.
• Instruct and aid patients in active and passive exercise, gait, ADL, and prosthetic training.
• Record and report patient response and coordinate care with the health care team.
• Educate patient, family, and caregivers on therapy within the scope of the PTA.
