Qualifications

• Current license to practice as a Physical Therapist Assistant in the assigned state.
• Graduate from an accredited Physical Therapy Assistant program as set by CAPTE.
• Current CPR certification.
• 1+ years of related PTA experience in home health care.
• If less than one year of experience, obtain approval from regional clinical leaders and participate in mentoring or a residency program.

Responsibilities

• Provides physical therapy services and interventions under the supervision of a licensed physical therapist.
• Observes, records and reports patient responses to treatment and changes in condition to supervising PT and clinical staff.
• Provides resources for future needs and promotes wellness and prevention.
• Documents supervisory activities in patient records.
• Monitors and documents response to physical therapy interventions.
• Participates in patient care conferences, in-services, chart audits and quality improvement activities.
• Maintains confidentiality of patient information.
• Makes follow-up care recommendations and coordinates with agencies for referrals.
• Performs other duties as assigned.
