Job summary

UnitedHealth Group is seeking a Physical Therapist to join our Home Health team. You will assess patients, develop a treatment plan, and implement interventions to enhance well-being in a home-health setting.

You will work with a multidisciplinary team, document progress, and ensure care aligns with state guidelines. A valid PT license and transportation are required; CPR is preferred or obtainable within 90 days of hire.

Qualifications

• Current Physical Therapy licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• CPR certification preferred or ability to complete within 90 days of hire.

Responsibilities

• Evaluates patient functional status and PT needs; assists in development of therapy plan of care.
• Provides PT services within scope of practice and plan of care; coordinates with health care team.
• Observes and reports patient response to treatment and changes to supervising nurse/physician.
• Treats patients to relieve pain and restore function.
• Instructs patient and family on plan of care, limitations and progress toward goals.
• Visits patients per Plan of Treatment; submits progress notes per policy.
• Completes patient evaluations and develops PT plan of care within state guidelines.
• Reports outcomes of evaluation, goals and proposed frequency of care.
• Function in any home setting irrespective of age, race, creed, color, sex or disability.
