Job summary

Qualifications

• Current and valid PTA license in good standing.
• Graduate of an accredited PTA program.

Responsibilities

• Travel to patients' homes to provide PT services under supervising PT.
• Review care plans, goals, and precautions before treatment.
• Provide skilled PT interventions per plan of care (exercises, gait, balance, transfer training).
• Educate patients and caregivers on exercises, mobility strategies, and fall prevention.
• Document daily treatment notes and patient progress.
• Communicate patient changes and progress with supervising PT.
