West Coast Wound & Skin Care is seeking a skilled Physical Therapist Assistant to provide in-home PT services throughout the San Diego area. You will work under a licensed Physical Therapist, delivering interventions per an established plan of care and educating patients and caregivers to improve mobility, safety, and independence.

The role emphasizes independent work in patients' homes, strong communication with the supervising PT, and meticulous documentation.

Qualifications

• Current and valid Physical Therapist Assistant license in good standing.
• Graduate of an accredited Physical Therapist Assistant program.
• Strong communication and interpersonal skills.
• Ability to work independently in a home-based care setting.

Responsabilités

• Travel to patients' homes to provide physical therapy under supervision.
• Review plans of care and goals before treatment.
• Provide interventions per plan including therapeutic exercises and neuromuscular re-education.
• Educate patients and caregivers on exercises, safety, and independence.
• Maintain timely clinical documentation and communicate progress to the supervising PT.
