Your Health Organization is seeking a Primary Care Physical Therapy Assistant to support licensed PTs in delivering rehabilitative services to homebound patients across facilities and homes. You will implement treatment plans, guide patients through exercises, document progress, and help coordinate discharge planning.

Formación

• Associate degree from an accredited Physical Therapy Assistant program.
• Current licensure or eligibility for licensure as a Physical Therapy Assistant in the state of practice.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for patients undergoing rehabilitation.
• Physical stamina and dexterity necessary to assist patients with mobility and perform therapeutic activities.
• Pelvic Health Experience and/or interest.

Responsabilidades

• Assist in implementing treatment plans under licensed PTs.
• Conduct therapeutic exercises with patients.
• Educate patients on proper rehabilitation techniques.
• Document patient progress accurately.
• Develop individualized treatment plans with PT input.
• Instruct and assist patients in performing therapeutic exercises.
• Monitor patients during therapy sessions.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate discharge plans and follow-up services with the care team.
