Alternate Solutions Health Network is seeking a licensed Physical Therapist Assistant to deliver skilled PTA services in patients’ homes under the supervision of a Physical Therapist. You will document progress, ensure safety, and maintain compliant care plans within Medicare guidelines.

The role emphasizes patient and family education, coordination with clinicians, and high-quality home-based therapy. A state PTA license and reliable transportation are required.

Qualifications

• Active Physical Therapist Assistant license in the state of employment.
• Minimum of one year of PTA experience in an acute setting.
• Home care experience preferred.
• Ability to communicate effectively with patients, families, physicians and coworkers.

Responsabilités

• Provide skilled PTA services in patients' homes under a Physical Therapist plan of care.
• Document patient progress and maintain compliant records per Medicare/agency guidelines.
• Educate patients and families on therapy techniques and use of equipment.
• Coordinate with scheduling and clinical staff to ensure timely care.
