Your Health is seeking a Primary Care Physical Therapy Assistant to support licensed physical therapists in providing rehabilitative services to homebound patients. You will perform visits in homes, ALFs, and ILFs, contribute to treatment plans, and document progress as part of a dynamic primary care team.

Qualifications

• Associate degree from an accredited PTA program.
• Current licensure or eligibility for licensure as a PTA in the state of practice.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for patients undergoing rehabilitation.
• Physical stamina and dexterity for mobility and therapeutic activities.
• Pelvic health experience and/or interest.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Conduct therapeutic exercises with patients and document progress.
• Educate patients on rehabilitation techniques and home programs.
• Develop individualized treatment plans and monitor patient response.
• Instruct and assist patients in performing exercises.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Coordinate with therapists and healthcare professionals to adjust plans.
• Assist in discharge planning and provide continued care instructions.
