Job summary

Your Health is seeking a Primary Care Physical Therapy Assistant to support licensed PTs and deliver rehabilitative care to homebound patients. You will help implement treatment plans, guide exercises, and document patient progress in homes, ALFs, and ILFs.

Two schedules are available for this full-time, salary-based role with a monthly bonus opportunity. Strong teamwork and empathy are essential to support diverse patient needs across facilities and home settings.

Qualifications

• Associate degree from an accredited Physical Therapy Assistant program.
• Current licensure or eligibility for licensure as a PTA in the state of practice.
• CPR certification and ability to work in home-care settings.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Educate and guide patients through therapeutic exercises and activities.
• Document patient progress and adjust plans as needed.
• Assist in discharge planning and coordinate follow-up services.
• Maintain a clean, organized, and sanitized therapy environment.
