Your Health is seeking a Primary Care Physical Therapy Assistant to join our growing home health team. You will work with licensed therapists to deliver rehabilitative services to homebound patients, developing and implementing treatment plans and educating patients on exercises.

Responsibilities include guiding patients through therapeutic activities, documenting progress, and coordinating with the care team to adjust plans. Position offers weekday hours, competitive compensation, and benefits.

Qualifications

• Associate degree from an accredited PTA program.
• Current licensure or eligibility for licensure in state of practice.
• CPR certification required.
• Strong interpersonal and communication skills.
• Ability to work effectively in a multidisciplinary team.
• Excellent organizational and time management skills.
• Compassion and empathy for patients.
• Physical stamina to assist patients with mobility.

Responsabilités

• Assist licensed physical therapists in implementing treatment plans.
• Conduct and supervise therapeutic exercises with patients.
• Educate patients on rehabilitation techniques and home exercises.
• Document patient progress and communicate with care team.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Assist with discharge planning and follow-up care recommendations.
