Job summary

UnitedHealth Group is seeking a Physical Therapist Assistant (PTA) to provide skilled home health care under the supervision of a licensed Physical Therapist. You will implement individualized treatment plans to improve mobility, strength, balance and independence at patients’ homes.

The role includes documenting responses to therapy, coordinating with caregivers and a multidisciplinary team, and supporting discharge planning with community resources.

Qualifications

• Current license to practice as a physical therapy assistant in the assigned state.
• CPR certification is required.
• One year of related PTA experience for home health is preferred; mentoring may be required for less experience.

Responsibilities

• Provide PT services under the supervision of a licensed Physical Therapist.
• Observe and document patient response to treatment.
• Assist with wellness and prevention resources and document supervisory activities.
• Record supervisory activities in the medical record.
• Monitor patient response and progress to therapy.
• Participate in care conferences and quality improvement activities.
• Maintain patient confidentiality.
• Refer to community resources and coordinate follow-up care.
