Job summary

DFW Home Health in Fort Worth/Azle area is seeking a Full-Time Home Health Physical Therapist Assistant (PTA) to join a passionate team. This PTA position covers the Azle area with a $2,500 bonus for full-time and relocation assistance possible.

As part of the Optum family, the role focuses on treatment, patient education, and adherence to the plan of care under supervision. Primary responsibilities include instruction, evaluation, and coordination of care with the health team to enhance patient

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver’s license and vehicle insurance or access to dependable vehicle or public transportation.
• Available to work a weekend rotation.
• CPR certification preferred or ability to obtain within 90 days of hire.

Responsibilities

• Follow state-specific laws governing physical therapy in home care; perform procedures defined by supervising PT; suggest changes to plan of care as needed.
• Instruct patients in exercise, gait, ADL, transfer, safety, and prosthetic training.
• Observe, record, and report patient responses; coordinate care with healthcare team.
• Instruct patient, family, caregiver, and others in therapy within PTA scope.
• Ensure capabilities to function in diverse home environments regardless of patient characteristics.
