Job summary

DFW Home Health in Fort Worth is seeking a Full-Time Home Health Physical Therapist Assistant (PTA) to join our caring team and support patients in the Azle area. You will implement treatment plans under supervision, educate patients and families, assist with mobility and functional activities, and coordinate care with a broader clinical team to promote independence and well-being.

This role offers a $2,500 bonus for full-time employees and a relocation bonus; you will provide hands-on therapy,

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in the state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.

Responsibilities

• Follow state-specific laws governing home care physical therapy; adhere to the supervising PT plan of care.
• Instructs patients in active and passive exercise, gait, ADLs, transfers, and safety.
• Observes and reports patient responses to treatment; coordinates care with the healthcare team.
