Job summary

DFW Home Health is seeking a full-time Home Health Physical Therapist Assistant to join our passionate team serving the Dallas area and Rockwall. You will perform treatment and provide exercise instruction and patient education as outlined in the plan of care.

This position offers a $2,500 bonus for full-time staff and relocation assistance. You’ll work within the LHC Group/Optum family, committed to caring, connecting, and growing together while delivering compassionate home health care.

Qualifications

• Current, licensed, registered, or certified to assist in the practice of physical therapy in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Available to work a weekend rotation.
• Preferred CPR certification or ability to complete within 90 days of hire.
• Ability to perform physical tasks required for patient care, including lifting, transferring, and assisting with mobility.

Responsibilities

• Follow state laws and the plan of care as defined by the supervising PT.
• Instruct patients in active and passive exercise, gait, ADL, transfer, and mobility.
• Observe and report patient responses; coordinate care with the healthcare team.
• Educate patient, family, caregiver on therapy within the PT scope.
• Provide care in home settings regardless of patient demographics.
