Zusammenfassung

Amedisys, part of the Optum family, is hiring a Physical Therapist to deliver skilled services at patients' homes. You will work with patients, caregivers, and a care team to promote mobility, restore function, and improve safety and independence in the home setting.

The role requires CAPTE-accredited PT training, active state licensure, CPR certification, and at least 1 year of PT experience in home health. A valid driver’s license and reliable transportation are also needed.

Qualifikationen

• Graduate from an accredited physical therapy educational program as set by CAPTE.
• Current license to practice physical therapy specific to the state.
• Current CPR certification.
• 1+ related physical therapy experience for home health care.

Aufgaben

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations (every 30 days, at minimum), modify interventions as needed, and perform discharge planning.
• Other duties as assigned.
