Job summary

Amedisys, part of the Optum family within UnitedHealth Group, is seeking a Physical Therapist to provide skilled home-based therapy that promotes mobility, independence, and safety in patients’ homes. You will collaborate with patients, caregivers, and an interdisciplinary team to support recovery and prevent hospitalizations.

Responsibilities include delivering therapeutic interventions, reassessments at 30 days, discharge planning, and ongoing care coordination.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• 1+ year of PT experience in home health or related field.
• If <1 year, regional clinical leaders’ approval and mentoring or residency participation.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days, modify interventions as needed, and discharge planning.
• Other duties as assigned.
