Job summary

Your Health is seeking a Primary Care Physical Therapy Assistant to join our growing team. You will work with licensed physical therapists to deliver rehabilitative services to homebound patients across facilities and homes in South Carolina, Georgia, and Florida.

The position is full-time, salary-based, with bonus opportunities and a strong emphasis on compassionate, patient-centered care in a multidisciplinary environment.

Qualifications

• Associate degree from an accredited Physical Therapy Assistant program.
• CPR certification required.
• Current licensure or eligibility for licensure in the state of practice.

Responsibilities

• Assist licensed physical therapists in implementing treatment plans.
• Conduct therapeutic exercises with patients and monitor progress.
• Educate patients on proper rehabilitation techniques.
• Document patient progress and coordinate care with the team.
• Maintain a clean, organized therapy environment and sanitize equipment.
• Support discharge planning and follow-up services.
