Job summary

Your Health in South Carolina seeks a skilled Primary Care Physical Therapy Assistant to support licensed physical therapists in delivering in-home rehab. The PTA will visit patients in homes, ALFs, and ILFs, developing treatment plans and guiding exercises, with time-efficient supervision and progress documentation.

The position involves multiple locations in the Charleston area, and offers a salary-based role with a monthly guaranteed bonus. CPR and state licensure eligibility are required.

Qualifications

• Associate degree from an accredited PTA program.
• Current licensure or eligibility for licensure as a PTA in the practicing state.
• CPR certification.
• Strong interpersonal and communication skills.
• Ability to work effectively as part of a multidisciplinary team.
• Physical stamina and dexterity for mobility and therapeutic activities. Pelvic health experience/interest.

Responsibilities

• Assist licensed PTs in implementing treatment plans.
• Conduct therapeutic exercises with patients.
• Educate patients on rehabilitation techniques.
• Document patient progress and adjust plans as needed.
• Develop individualized treatment plans and discharge instructions as part of care.
• Maintain a clean, organized therapy environment and coordinate with other staff.
