Job summary

The role requires a CAPTE-accredited degree, current state licensure, CPR certification, and 1+ year of home health PT experience; mentoring options available if needed.

Qualifications

• Graduate from an accredited physical therapy program as set by CAPTE
• Current license to practice physical therapy in the assigned state
• Current CPR certification
• 1+ year of PT experience in home health care or equivalent
• If less than 1 year, regional clinical leaders must approve and mentoring/residency may be required
