Job summary

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• Minimum 1 year of related PT experience in home health or approval with mentoring for less than 1 year.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement, activity and function.
• Provide interventions, conduct reexaminations every 30 days minimum, modify interventions as needed, and perform discharge planning.
• Other duties as assigned by the interdisciplinary care team.
