Amedisys, part of the Optum family, is hiring a Physical Therapist - HAC to restore function and improve mobility for home-based patients. You will deliver skilled PT services under a physician-directed plan, coordinating with an interdisciplinary team.

Responsibilities include patient assessments, treatment planning, ongoing re-evaluations, and accurate documentation while maintaining compliance with clinical standards. Valid state license and CPR are required.

Qualifikationen

• Graduate of an accredited physical therapy program (CAPTE accredited).
• Current, unencumbered license to practice physical therapy in the assigned state.
• CPR certification and valid driver's license with reliable transportation.
• 1+ years of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program.
• 1+ years of experience in clinical assessment, documentation, and care coordination.

Aufgaben

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
