Job summary

UnitedHealth Group's Amedisys home health team is seeking a Physical Therapist to deliver skilled PT services in patients’ homes, promoting mobility, restoring function, and improving safety and independence through personalized care.

You will collaborate with patients, caregivers and an interdisciplinary team, perform 30-day reexaminations, plan discharges, and participate in a comprehensive benefits package. Licensure and CAPTE-accredited degree required; CPR certification essential.

Qualifications

• Current license to practice physical therapy in the state you’re assigned to
• Graduate from a CAPTE-accredited physical therapy educational program
• 1+ years related physical therapy experience for home health care
• If less than one year of experience, approval from regional clinical leaders and mentoring/residency participation
• Current CPR certification

Responsibilities

• Help patients maintain, restore and improve movement, activity and function
• Provide interventions, conduct reexaminations every 30 days (at minimum), modify interventions as necessary, and perform discharge planning
• Other duties as assigned
