Job summary

UnitedHealth Group, including Amedisys home health services, is seeking a licensed Physical Therapist to provide in-home therapy. You will help patients improve mobility, balance, and independence through skilled interventions and reexaminations, collaborating with caregivers and a multidisciplinary team.

Responsibilities include maintaining and restoring movement, planning discharge, and adapting therapies as patients progress.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited physical therapy program.
• At least 1 year of related home health PT experience, or approval if less than 1 year.
• If under 1 year of experience, mentoring and/or residency participation may be required.
• Current CPR certification.

Responsibilities

• Help patients maintain, restore and improve movement and function.
• Provide skilled PT interventions, conduct 30-day reexaminations, modify plans, and discharge planning.
• Collaborate with patients, caregivers, and an interdisciplinary team.
• Perform other duties as assigned.
