Job summary

UnitedHealth Group, in collaboration with Amedisys as part of the Optum family, is seeking a Physical Therapist to restore function and mobility for home-based patients. The HAC role provides skilled therapy under a physician-directed plan of care within an interdisciplinary team.

Responsibilities include comprehensive assessments, developing plans, delivering interventions, ongoing reassessments, care coordination, and meticulous documentation.

Qualifications

• Current, unencumbered license to practice physical therapy in the assigned state.
• Graduate of an accredited physical therapy program (CAPTE accredited).
• 1+ years of experience in physical therapy; OR less than one (1) year with participation in an approved residency or mentorship program.
• Current CPR certification.
• Driver's License and access to reliable transportation.

Responsibilities

• Conduct comprehensive patient assessments and develop individualized physical therapy plans of care in collaboration with physicians.
• Provide skilled physical therapy interventions and modify treatment plans based on patient progress and clinical needs.
• Perform ongoing reassessments, including weekly evaluations, to support care planning and physician coordination.
• Collaborate with interdisciplinary teams to coordinate care and communicate patient progress and condition changes.
• Maintain accurate, timely documentation in compliance with clinical, regulatory, and documentation standards.
