Job summary

UnitedHealth Group's Amedisys home health team in Virginia is seeking a licensed Physical Therapist to deliver skilled PT services in patients' homes, focused on mobility, function, safety and independence. You will collaborate with patients, caregivers, and an interdisciplinary team to support recovery and reduce hospitalizations.

Responsibilities include maintaining and improving movement, performing reexaminations every 30 days, discharge planning, and coordinating care.

Qualifications

• Current license to practice physical therapy in the assigned state.
• Graduate from a CAPTE-accredited PT program.
• 1+ year of home health PT experience or approval from regional clinical leaders if less than one year.
• Current CPR certification.

Responsibilities

• Deliver skilled physical therapy to patients in home settings.
