Job summary

Amedisys, part of the Optum family, is seeking a Physical Therapist to provide skilled therapy services in patients' homes to promote mobility, restore function, and enhance safety and independence. You will work with patients, caregivers and the care team to support recovery and improve quality of life within the home setting.

Responsibilities include 30-day reexaminations, intervention modifications, discharge planning, and collaboration with the care team.

Qualifications

• Graduate from an accredited physical therapy educational program per CAPTE.
• Current license to practice physical therapy in the assigned state.
• Current CPR certification.
• 1+ year of PT experience in home health or equivalent, with mentoring if less than 1 year.

Responsibilities

• Provide skilled physical therapy services in patients' homes to promote mobility, restore function, improve strength and balance, and enhance safety and independence.
• Perform reexaminations at minimum every 30 days, modify interventions as necessary, and conduct discharge planning.
• Collaborate with patients, caregivers and the interdisciplinary care team to support recovery and reduce hospitalizations.
