Job summary

Ohio Living in Akron is seeking a licensed Occupational Therapist for home health and hospice services. You will evaluate and treat geriatric patients in their homes, develop care plans, and educate families to support independence and safe activities. 

The role requires an active Ohio OT license, strong communication, and the ability to work within an interdisciplinary team to ensure high-quality patient care.

Qualifications

• Current unencumbered license for the state of Ohio to practice as an Occupational Therapist.
• Experience in home health and geriatric care preferred; new graduates welcome.
• Ability to read/write English and operate EMR documentation.

Responsibilities

• Perform occupational therapy evaluations and treatments in patient homes as ordered by physicians.
• Develop and implement Plan of Care and discharge planning with the team.
• Educate patients/family on exercises and safety, promote independence.
• Maintain patient records and collaborate with interdisciplinary team.
• Drive to patient homes and adhere to safety and compliance guidelines.
