Ohio Living is seeking an Occupational Therapist to perform evaluations and treatments for patients in home health settings, following physician orders and state/federal guidelines to ensure the highest quality care. You will conduct OASIS assessments, plan of care revisions, and educate patients and families for safe, independent home function.

Requires an active Ohio OT license; one year of home health or geriatric care experience preferred.

Formación

• Current unencumbered Ohio OT license.
• Experience with geriatric patients preferred; new graduates welcome.
• Valid driver’s license and acceptable driving record.
• Able to read, write, and speak English clearly.

Responsabilidades

• Care Management and Service Delivery – 60% of duties including home visits and plan of care formulation.
• Administration and Reporting – 20% with EMR and chart documentation.
• Quality, Compliance and Risk Management – 10% ensuring standards and confidentiality.
