Job summary

Ohio Living is seeking a dedicated Occupational Therapist to provide home health OT services across our life plan communities in Ohio. You will assess patients, develop plans of care, and collaborate with an interdisciplinary team to support residents' functional goals.

Requirements include an active Ohio OT license; hospital, LTC, or home health experience is preferred. New graduates are welcome, with travel involved to patient homes as needed.

Qualifications

• Current unencumbered Ohio OT license is required.
• Hospital, long-term care, or home health experience preferred.
• New graduates welcome.
• Experience with geriatric OT care is required.

Responsibilities

• Initiates physician orders and treatment plans as patient conditions require.
• Performs home-based OT evaluations and treatments.
• Educates patients and caregivers on exercises and self-care.
• Participates in discharge planning to ensure continuity of care.
