Ohio Living seeks a licensed Occupational Therapist to deliver care in home health and long-term care settings. You will assess patients, develop plans of care, and coordinate with physicians to implement orders, ensuring safe, effective therapy in patients' homes.

The role requires current Ohio OT license, up to one year of hospital, long-term care, or home health experience; new graduates welcome. English proficiency, valid driver's license, and liability coverage are required.

Formación

• Current unencumbered Ohio OT license.
• New graduates welcome; one year hospital/long-term care/home health experience preferred.
• Must read, write, speak English; valid driver's license and acceptable driving record; insurable under liability policies.

Responsabilidades

• Care management and service delivery in patient home settings; initiates physician orders and treatment plans; participates in OASIS assessments and discharge planning to improve function.
• Maintains current patient records using EMR and paper documentation per guidelines.
• Coaches and collaborates with the health care team to ensure continuity of care and safety; contributes to quality, compliance and risk management.
