Job summary

Ohio Living in Ohio is seeking an Occupational Therapist to perform evaluations and treatments per physician orders, ensuring high-quality care in the patient’s home or care setting.

You will develop Plan of Care, participate in discharge planning, educate patients and families, and collaborate with an interdisciplinary team to improve function and safety for patients returning home. New graduates welcome; OH license required.

Qualifications

• Current unencumbered license for the state of Ohio to practice as an Occupational Therapist.
• One year of experience as an OT in healthcare settings preferred; new graduates welcome.
• Knowledge of treatment modalities.
• Must read, write, speak and understand English.
• Valid driver's license and insurable as per policy.

Responsibilities

• Perform occupational therapy evaluations and treatments as ordered by physicians.
• Develop and implement patient OASIS assessments and discharge planning.
• Conduct home visits to improve function and safety and help patients return home.
• Educate patients and families on exercises, precautions and self-care.
• Collaborate with an interdisciplinary team to ensure continuity of care.
• Maintain compliance with company policies and regulatory standards.
