Job summary

Ohio Living is seeking an Occupational Therapist for home health and hospice services in the Greater Cleveland area. You will conduct OT evaluations and provide in-home treatments, educate patients and families, and collaborate with an interdisciplinary team to ensure high-quality, safe care.

The role requires an Ohio license, a valid driver’s license, and the ability to travel to patient homes. New graduates are welcome to apply as part of our committed care team.

Qualifications

• Current unencumbered Ohio OT license required.
• One year experience in hospital, long-term care facility, or home health preferred; new graduates welcome.
• Experience in geriatric care and occupational therapy.
• Must read/write/speak English.
• Valid driver's license and insurability under liability policies as per insurer.

Responsibilities

• Care management and service delivery, including initiating orders and formulating treatment plans with physicians.
• Perform OT evaluations and home-based treatments per physician orders.
• Educate patients and families on exercises, safety, and self-care activities.
• Participate in interdisciplinary team meetings and discharge planning.
• Maintain patient records in EMR and paper charts per regulations.
• Provide guidance to Certified Occupational Therapy Assistants and support staff.
