Ohio Living seeks an Occupational Therapist to provide home health care services across our life plan communities in Ohio. The role focuses on care management, treatment planning, and collaboration with physicians and the interdisciplinary team.

New graduates are welcome, with preference for those experienced with geriatric OT and various treatment modalities. Strong communication and safety awareness are essential to support patients returning to their highest level of function.

Competenze

• Current unencumbered license for the state of Ohio to practice as an Occupational Therapist (OT).
• One year experience as an Occupational Therapist in a hospital, long-term care facility, or home health agency preferred.
• New graduates welcome.
• Experience in care of geriatric and occupational therapy patients required.
• Knowledge of treatment modalities.

Mansioni

• Initiates orders and treatment plans with physicians as patient condition requires.
• Performs OT treatments and evaluates patients in home health setting.
• Recommends modifications to the Plan of Care based on progress.
• Educates patients and families on exercises, precautions, and self-care.
• Participates in interdisciplinary team meetings to ensure continuity of care.
• Coordinates discharge planning with staff, family, and physicians.
• Maintains clean, safe treatment areas and proper documentation.
