Job summary

Ohio Living seeks a dedicated Occupational Therapist to perform evaluations and treatments in accordance with physician orders and state guidelines, delivering high-quality care to patients in home health and facility settings.

You will initiate care plans, participate in OASIS assessments, discharge planning, education for patients and families, and collaborate with an interprofessional team to promote function and safety.

Qualifications

• Current unencumbered license to practice OT in Ohio.
• One year of OT experience in hospital, long-term care, or home health; new graduates welcome.
• Experience with geriatric patients and OT care.
• Must read, write, speak English.

Responsibilities

• Initiates physician contact for orders and treatment planning.
• Participates in development and implementation of OASIS assessments and discharge documentation.
• Performs OT treatments as prescribed, in patient home or care setting.
• Educates patients, families, and health team on exercises and self-care.
• Communicates progress and discharge plans with patient, family, and physicians.
• Conducts home visits to implement the Plan of Care and improve function.
• Participates in discharge planning to ensure continuity of care.
• Maintains clean, sanitary treatment areas and equipment.
