Job summary

Qualifications

• Must have an active Ohio OT license.
• One year experience in hospital/long-term care/home health preferred.
• English language proficiency required.

Responsibilities

• Performs OT evaluations and treatments as ordered by physicians.
• Develops and revises Plan of Care for home health patients.
• Educates patients and families on exercises and self-care.
• Participates in interdisciplinary team meetings for care continuity.
• Maintains accurate patient records in EMR and chart documentation.
• Ensures safety and quality by adhering to standards and guidelines.
