VitalCaring Group is seeking a Home Health Occupational Therapist to help patients regain independence in their homes. You will partner with patients, families, and interdisciplinary teams to deliver personalized, high-impact care.

Responsibilities include evaluating functional status, assessing home safety, and developing plans with clear goals. Requires NBCOT, state license, CPR, and a valid driver’s license. Strong clinical judgment and communication are essential.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification and state licensure required.
• Current CPR certification and valid driver's license.

Responsibilities

• Evaluate patients’ functional status, including mobility and daily activities.
• Assess home environments for safety and recommend modifications.
• Develop individualized treatment plans with measurable goals.
• Provide interventions to restore function and retrain daily living activities.
• Collaborate with care team and participate in discharge planning.
