Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to empower patients to regain independence at home. You will work closely with patients, families, and the care team to deliver personalized, high-impact occupational therapy.

Responsibilities include evaluating functional status, home safety assessments, developing treatment plans, training in daily activities, supervising COTAs, and coordinating care to ensure smooth transitions.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of 1 year home health experience preferred.

Responsibilities

• Evaluate patients’ functional status and daily living activities.
• Assess home environments for safety and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with measurable goals to improve independence.
• Provide therapeutic interventions to restore function and retrain in daily living activities.
• Supervise COTAs in accordance with state regulations and company policy.
• Coordinate with physicians and interdisciplinary team members for seamless care.
• Participate in discharge planning to support transitions and continued progress.
