Job summary

VitalCaring Group is seeking a dedicated Occupational Therapist for field-based home health care. You will evaluate functional status, create personalized plans, and collaborate with families and interdisciplinary teams to improve daily living activities in patients’ homes.

The role emphasizes safety, education, and seamless care coordination, with opportunities to mentor staff and participate in discharge planning. A valid license and proven home health experience are preferred.

Qualifications

• Graduate of an accredited Occupational Therapy program
• NBCOT certification
• Current state licensure as an Occupational Therapist
• Minimum of 1 year clinical experience; home health a plus
• Current CPR certification
• Valid driver’s license, reliable transportation, and auto insurance

Responsibilities

• Evaluate patients’ functional status including mobility, strength, coordination, communication, and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with measurable goals to improve independence.
• Provide therapeutic interventions to restore function and retrain daily living activities.
• Fabricate splints and train patients in adaptive/orthotic device use.
• Support patients returning to work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on plans, safety, and ongoing care.
• Collaborate with care team to coordinate services and ensure patient-centered care.
• Participate in discharge planning to support transitions and progress.
