Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to help patients regain independence and function in the home. You will assess, plan, and implement individualized treatment, collaborating with families and the care team for high-impact care.

Requirements include NBCOT certification, current state OT licensure, CPR, and a valid driver's license with reliable transportation. This is a full-time field-based role with comprehensive benefits.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.
• Minimum of one year of clinical experience preferred (home health experience a plus).
• Current CPR certification.
• Valid driver’s license with reliable transportation.

Responsibilities

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with measurable goals to improve independence and quality of life.
• Provide therapeutic interventions to restore function and retrain in activities of daily living.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure patient-centered care.
