Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to help patients regain independence and enhance daily living in their homes. You will work with patients, families, and a multidisciplinary team to deliver personalized, high-impact care.

The role emphasizes safe home evaluations, adaptive equipment, and functional retraining, with leadership over COTAs and potential supervision of home health aides where required.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification.
• Current state licensure as an Occupational Therapist.

Responsibilities

• Evaluate patients' functional status including mobility, strength, coordination, communication, and daily activities.
• Assess home environments to identify safety risks and recommend modifications or adaptive equipment.
• Develop individualized treatment plans with clear, measurable goals to improve independence and quality of life.
• Provide therapeutic interventions to restore function, including retraining in activities of daily living.
• Fabricate splints and train patients in adaptive, prosthetic, and orthotic device use as appropriate.
• Support patients returning to work through functional and work capacity evaluations.
• Educate patients, families, and caregivers on treatment plans, safety, and ongoing care needs.
• Collaborate with the care team to coordinate services and ensure patient-centered care.
