Job summary

VitalCaring Group is seeking a Home Health Occupational Therapist to help patients regain independence at home. You will evaluate function, create treatment plans, and collaborate with families and the care team to deliver patient-centered care.

This field-based, full-time role requires NBCOT certification, active OT license, CPR, and reliable transportation. You’ll supervise COTAs and contribute to discharge planning while advancing clinical practice.

Qualifications

• Graduate OT program and NBCOT certified with active state license.
• CPR certification and valid driver's license required.
• Minimum 1 year home health or clinical experience preferred.

Responsibilities

• Evaluate patients’ functional status including mobility, strength, and daily activities.
• Assess home environments for safety risks and recommend modifications.
• Develop and monitor individualized treatment plans with measurable goals.
• Provide interventions to restore function and retrain in daily living activities.
• Fabricate splints and train use of adaptive devices as appropriate.
• Support return-to-work through work capacity evaluations.
• Educate patients, families, and caregivers on plans and safety.
• Collaborate with the care team for seamless patient-centered care.
• Participate in discharge planning to support continued progress.
• Document care in the EMR and communicate plans with physicians.
