Job summary

VitalCaring is seeking an Occupational Therapist for home health, offering field-based, full-time, patient-centered care. You will evaluate function, create individualized plans, and collaborate with families and interdisciplinary teams to support independence at home.

You will help patients regain daily living skills, advocate for safety in home environments, and participate in discharge planning to ensure smooth transitions within a compassionate care team.

Qualifications

• Evaluate patients’ functional status, including mobility, strength, coordination, communication, and daily activities.
• Determine safety risks in home environments and recommend modifications or adaptive equipment.
• Create individualized treatment plans with clear, measurable goals to improve independence.
• Provide therapeutic interventions to restore function and retrain in daily living activities.
• Fabricate splints and train patients in adaptive, prosthetic, and orthotic devices as appropriate.
• Support patients returning to work through functional/work capacity evaluations.
• Educate patients, families, and caregivers on plans, safety, and ongoing care needs.
• Coordinate with care team to ensure seamless, patient-centered care.
• Participate in discharge planning to support transitions.
