Job summary

VitalCaring seeks a dedicated Occupational Therapist to join our Home Health team. You will evaluate patients at home, develop individualized plans, and work with families to restore independence and safety. Collaboration with interdisciplinary teams ensures seamless, high-quality care.

The role requires OT licensure, NBCOT certification, CPR, and a valid driver’s license. Prior home health experience is a plus, with opportunities for professional growth in a supportive, caring culture.

Qualifications

• Graduate of an accredited Occupational Therapy program.
• Current NBCOT certification is required.
• Current state licensure as an OT is required.
• Minimum of one year of clinical experience preferred (home health is a plus).
• Current CPR certification and valid driver’s license are required.

Responsibilities

• Evaluate patients
• Assess home environments for safety risks and recommend modifications
• Develop individualized OT treatment plans with measurable goals
• Provide interventions to restore function and retraining in daily living activities
• Supervise COTAs in line with state regulations
• Educate patients and families on treatment plans and care needs
• Coordinate with care team to ensure seamless, patient-centered care
• Participate in discharge planning and transitions
